All Saline Golfers MUST submit evidence of a completed physical examination prior to the beginning of the season.

In addition, all golfers must return signed forms (Athletic Rules and Regulations, Medical Consent/Emergency Card, Field Trip Consent, and Sportsmanship Pledge) prior to competing in the first match.

You may also pick up most forms in the Saline High School office, 7190 N. Maple Road, Saline, MI between 8:00 a.m. and 3:30 p.m. on school days. During the summer months, forms are available in the Athletic Office located in the far west end of the high school building. Contact the Athletic Secretary at 734-429-8054 for more information.
